How we meet: Once your appointment is scheduled, you will be sent a Google Meets link. Please use this link to join the meeting on your scheduled date and time.

Once the appointment is scheduled, we will email you a Nutrition History form to fill out so that Autumn can better personalize your action steps to your history, preferences, and goals. 

Cancellation Policy: Appointments can be rescheduled up to 24 hours in advance without penalty. Within 24 hours of your appointment, you will be charged in full if the appointment is missed, changed or cancelled. 

We do not accept insurance or provide super bills.
